The acute care service includes:
The Continuing Complex Care service consists of 10 beds designated for complex continuing care.
Care is provided by a multidisciplinary team of family practice physicians, anaesthetists, general surgeons and visiting/consulting specialists; registered nurses, registered practical nurses, physiotherapists and occupational therapists; pharmacist, dietician; social worker/discharge planner, diabetes educator, mental health and addictions counsellors and a patient navigator.
Please consider appointing a family spokesperson. This person can take on the task of calling the hospital for information and passing news on to the rest of the family or friends. This will help limit the calls to the nurses during your admission. Please discuss this with your nurse when you are admitted.
You will be supplied with a Discharge Planning Booklet on admission. This booklet will help you and your family prepare for your discharge. It will also guide you in helping the health care team get you ready for discharge.
Allergies: tell your nurse about any allergies that you have to medication or food allergies or intolerances.
Medications: bring in all of your medications, in their original containers, or a list of medications you are currently taking. Once medication has been received, reviewed and recorded, they may be taken home by your family. Some of your medications, if not stocked by the hospital, may be administered by your nurse. Medications not taken home are placed in safekeeping.
Pain and other symptoms: knowing about your pain and other symptoms will help your health care providers to determine a diagnosis and the best course of treatment.
